After you buy a hospice, check the charts before the next recert is billed

When a deal closes, you take on every open chart. The next recerts and monthly claims are billed under your ownership, often before the acquired team has learned your standards. We check those charts first. This is most useful when the hospice you bought does not have its own QA team yet.

What we check

  • Each upcoming recertification (recert) chart, against what a Medicare reviewer asks for first: does the record support a terminal prognosis for the days billed? At CGS, one of the Medicare contractors that review hospice claims, this was the reason for 53.6% of denials in targeted reviews from April to June 2026.
  • Election statements and addenda for current patients, against 42 CFR 418.24. Invalid notices of election were the next most common reason at CGS, at 26.4%.
  • Every finding cites the page in the chart, and a US RN signs off.

How it works

  1. We sign a business associate agreement (BAA) with the hospice you acquired.
  2. US hospice nurses review charts before each recert is billed, on a schedule you set.
  3. Your clinical team gets a gap list with page references and decides what to do.

What we never do

  • Decide eligibility. The hospice physician decides.
  • Write or change a chart. Any correction is a dated late entry by the hospice's own staff.
  • Soften findings. The report says what the record shows.
  • Contact patients or families.
