Privacy notice
This notice covers ready.kogsy.ai and its meeting request form. It does not cover patient information that KOGSY Ready handles for customers under a business associate agreement.
Who handles your details
HG Connect UG (haftungsbeschränkt), Spatzenweg 17, 85238 Petershausen, Germany, controls the business details you send through this site. Email hg@kogsy.ai with privacy questions.
What we receive
The meeting form asks for your name, work email, organization, role, provider type, state, and your agreement to be contacted about the request. You can optionally say whether your organization is under Medicare review. If you email us, we receive your address and message. Please do not send patient information.
Meeting requests
When you send a meeting request, we collect your name, work email, organization, role, provider type, state, and, if you choose to answer, whether your organization is under Medicare review. We use these details only to reply to you and to discuss a possible pilot. The form is processed by our website host, Netlify, Inc. We keep requests for 24 months after the last contact if they do not lead to a customer relationship. You can ask us to delete your request at any time by writing to hg@kogsy.ai. Please do not send patient information through this form.
Why we use it
We use these details to answer your inquiry and discuss a pilot. For GDPR purposes, we process requests made before a possible contract or rely on our legitimate interest in replying to business inquiries, as applicable.
Service providers and location
Netlify hosts this site and receives form submissions. We also use an email service to read and reply to inquiries. These services may process data outside the European Economic Area.
Fonts and site measurement
IBM Plex and Montserrat fonts are hosted on this site, so loading them does not send a request to Google. We use Plausible Analytics to understand overall website traffic. Plausible receives page paths, the sites that sent visitors, and basic browser, device, and approximate location information. It does not set cookies or store raw IP addresses or persistent visitor identifiers. Visitor data is processed in the EU. See Plausible's data policy. We do not use advertising trackers or our own cookies.
How long we keep it
When an inquiry does not lead to a customer relationship, we keep it for 24 months after the last contact. Records related to a customer relationship may be kept longer when needed for that relationship or required by law.
Your choices
Contact hg@kogsy.ai to ask for access, correction, or deletion of your form details. People covered by GDPR may also object to certain processing and complain to a data protection authority. US visitors may contact us with the same requests.
